This Pull Request cleans up the Bug issue template:

  • remove the TODO prompt since we don't yet have a tool to collect environment info
  • removed unhelpful placeholder that is partially visible and poorly formatted
  • removed extraneous whitespace in the template

LGTM. Weird that the placeholder text doesn't respect the new lines. I would have assumed it did.
