First workable release

• I am still working on the accounts UI functionality so that may change intermittently
• the plugin may not actually correctly identify things as open or not, yet.
• in terms of using it and seeing how the steps go, it works.
• Also it will link to request pages that do not necessarily show anything yet
• as soon as I get accounts sorted, the request pages will be up

This is for use only in basic user testing.
