This is a combination of #101 and #104, which I needed to close to clean up our fork.
I've combined them, as having tested in a real app you really need to be able to have multiple system prompts in order to set effective cache break points.
Rationale and choices made are covered in the description of those PRs.
The only changes made versus those PRs:
Once this is merged, I will submit a PR for Anthropic PDF processing.
