Issue 625 - Remove canonicalization of addresses

Acceptance Criteria:

• Should be able to enter an invalid address like "1 Foo St Cambridge MA 12345" and proceed to the survey visit page and the home is saved.
• Should be able to enter an address, proceed to the survey visit page, go back, and enter the same address, and again proceed to the survey visit page.
• Should (still) be able to create a home through the admin interface

Side note - if we are concerned with downtime / some temporary "column not found" errors, I could separate out the database migration, but I assume it's a non-issue
