v0.1.9: Remove usage of gatk-framework; replace with bcftools

Removes usage of older GATK3 gatk-framework, replacing with
modern bcftools equivalents. gatk-framework is no longer distributed
with bcbio. bcbio/bcbio-nextgen#2412
