Fixes the broken toolchain.md link by removing the .md extension and ./ prefix.
I also changed all of the other internal documentation links with .md extensions for consistency.
There was a syntax error caused by a line break under PR 1406 in user-changes.md that broke formatting of old changelogs, so I fixed this as well.
Tested with local jekyll build.
