Various fixes and minor changes

- Moved to 0.4.0
- Added Example.Line
- Renamed ProtocolPeer.SendAsync(CancellationToken) to ProtocolPeer.FlushAsync(CancellationToken)
- Cleared up end of stream logic
- Fixed bug which would prevent peers fully disconnecting if a observer was subscribed
