1. Fix color coding options
2. Added text on outcome results
3. Fixed output for durable toilet outcome
4. Fixed typos in community form and order of options on soil type

Additional updates

1. Upgrade akvo-react-form to v2.5.4
2. Fix other outcome auto fields in HH form
3. Fix invalid prefilled answer in HH form
4. Update init db
