Add moderation tools #23

Several people have requested the ability for the bot to do mod operations, which this new cog provides by storing them in an sqlite database.

All requested commands implemented
Logging command with pagination
Add reloading commands
Better error handling
Database tests
Move everything out of bot.py to allow for dynamic reloading of bot without clearing cache

should have been done elsewhere

Put all commands in main class so reloading works correctly
