[LUPEYALPHA-877] Send an email to the practitioner when the provider submits a claim

• When the PROVIDER sees the /confirmation screen an email is triggered from NOTIFY to the PRACTITIONER
• The email is sent to the PRACTITIONER using the employee-email that was capture in the provider journey
• Includes the claim reference and email address in the URL to the practitioner journey start page (yet to be built)
