Ethics: Add ethical issues report to DAMAP

Description
From the RDA-Common-standard. This report is not yet in DAMAP, eventhough it is in the common standard. Med Uni Graz requires this, therefore we should implement it. Find a way to add it to our data model. Also ensure that it is in the form of an URI. It would also be a good idea to only allow this field when yes was chosen in this question:

But run this by Christiane and Tomek first, since this yes/no question is not part of the common standard.

Proposed solution
Expand our datamodel with the ethical issues report. Also make sure that the field has the form of an URI and is only filled when yes was chosen for the above question (ask Tomek and Christiane first). You also have to add this new value to our export templates, discuss with Christiane how to do this.
