I'm not sure if this is the right place to drop this. I love this plugin - it's helped me a great deal. I had two requests for consideration.

Would it be possible have it use a template file (as opposed to a template text box) for index file generation? I know some plugins do this but I don't know how complex the effort is.
Can you add a way to append a prefix or suffix to the file name "index-(foldername).md, (foldername)-index.md, etc)
