De-dupe audio before attaching to $story and pushing to NPR

Since audio can be added to NPRML via 2-3 methods, and we can assume that any given audio should be attached to a given story only once, we should first collect all audio candidates, de-dupe them (using URL as key), and then attach them to $story.
