New Lab - Request More Info - Error #544

Describe the bug
Mandatory questions on a new lab, will make it easier to filter and make sure labs are adding the correct information

To Reproduce
Steps to reproduce the behavior:

Go to Lab on the backstage
Click on Request more info
Scroll down to send
See error:
Uh oh. 3 problems need(s) fixing before you can continue:
Blurb can't be blank
Description can't be blank
Phone can't be blank

This shows because the creator of the lab didn't fill the form and it let send the request.

Expected behavior
All questions must be answered under the add new lab form in order to submit your request as a new lab.

Approving Labs, sending request more info to lab, updating lab.
