Error Adding a phone

I'm getting this error after pressing the "add phone"

Thanks for the prompt response. Here's our FreePBX version:
PBX Version:16.0.21.9
PBX Distro:12.7.8-2208-2.sng7
Asterisk Version:16.27.0

Try using the fork that i made, there were a few changes in fpbx 15 and 16 that excalibur have not updated.. i have those updates in my fork.

after adding the phone information. How can we point the Polycom device to properly provision with the FreePBX server?

Use DHCP Option 160, and set the value to something like http://pbxuser:pbxpass@ipofbox/polycom
the pbxuser/pbxpass is set under the polycom network menu.
