In unsupported countries, after deploying the matrix synapse service overseas, you can receive push messages. ElementX on ios cannot call callkit. Click the video call button, and only a push message for the video call will pop up. If you enable VPN on the ios side, you can Wake up the callkit. What’s interesting is that I deployed the signal agent on the synapse server. At this time, the ios element X can call the callkit. When the ios element calls the callkit call page, the Android element X's call becomes a push notification

I found a very interesting problem about the video call callkit of elementX. When my ios language is set to Simplified Chinese, CallKit cannot be called normally. When my speaking language is set to Traditional Chinese or English, the callkit can be called normally. I I can't confirm whether it is an issue with iOS. There is another very interesting thing. When I deployed the signal agent on the synapse server, iOS can call callkit normally no matter what language is used. I used two devices (iPhone 15 pro max ios17.7.2 iphone xsmax ios 16.7) The tests all have the same problem, the old version of elementX and the latest elementX. Is this the traffic strategy of elementX? I cannot be sure.

I can't find any specific Apple page detailing it, but if by this you mean China, then the CallKit framework is banned there so none of what you describe above is particularly unexpected.
