As a new CBO user, I want to create an account that allows me to access the CBO Referral App so I can manage referrals.

Acceptance criteria:

"Create an account" screen and flow.
New user is approved by admin.
New user is added to Azure Active Directory.
New user can use their new login credentials to log into CBO Referral App.
