Patient should be able to view the referral using MyCarePlanner (SMART on FHIR launch using aP credentials).

Do we also want the patient to view Referral Task Status? If so, we'll need to connect it to the Social Care Hub.

Heard from AC that aP does not have patient login and therefore there's no ability for the patient to log into MyCarePlanner.
