[Dashboard] Apply dashboard filters for Comment card

AC:

• Add filters for subject type, program and encounter type in App designer on report_card like done for previous standard report card types
• The above filters and all custom filters need to work
• Should be able to select any colour for the report card and it should get applied.
• Apply all the custom filters(subject types and custom filters) on the comments
• currently we are filtering comments of only open comment threads - retain the functionality.
• Eg: When program p and program encounter e filters applied,
  'Comment' card need to show individuals enrolled in p and having encounter e(completed or scheduled) and having comments in open comment threads.

Technical:

subject_id in comment table would form the bridge to filter the results
