iplog to log all names the client uses

listen for userinfochanged/rename events.
expose rcon command to show all names used by an IP, and all IPs used by a name (except for Padawan?)
Could show in a tree view, like:

]iplog Foo
Foo has been used by:
    255.255.255.255

]iplog 255.255.255.255
255.255.255.255 has gone by these names:
    Bob
    Jerry
    Tom
        Also used by:
        > 123.123.123.123
        > 69.141.63.3
    Dave
