Steps to reproduce

1.some players like 5+ players with listen server
2.respawn
3.sometime crash

on that note, you have a lot of mods installed so there's a decent chance this issue is stemming from one of the mods you have installed. Try disabling all of them temporarily and see if that fixes the issue for you.
