Combine operator and radcheck accounts into a unified account

Currently operators and users have their own tables. To allow for future endeavors with Radium, we need to combine the records. This will allow 1 user to have multiple radius accounts, and allow for a clean way to whitelist mac addresses without cluttering up the user list.
