Reorder the Monitoring Information section

Since there will now potentially be two follow up tables if the answer to both these questions is yes - instead of having the two questions side by side in the contained can we go one on top one on the bottom?

Then when the top question is responded yes, the table will populate below as we have it now. The second question will then be underneath this first table, and will populate another identical table below if the second question is also yes.
