Add Weekly Comment Modals

Description: Have a weekly comment modal populate that allows supervisors to give standardized feedback for weekly performance.

**NOTE: The weekly comment box design needs to be updated. There should be a "weekly performance evaluation" button at the top of the spreadsheet in line with the search and date boxes that once clicked populate this modal! **

**Acceptance Criteria: **

• Allow user to choose between 5 options, one of them being an other options where they can leave custom feedback.
• User should not be able to hit submit unless a choice is selected
• Hitting submit should eventually auto populate a comment with whatever was selected on the modal, so make sure to save this data somewhere!
• Cancel should bring user out of the modal
